SUMMARY The social worker will demonstrate support of Roane General Hospital values by acting as an advocate to help clients gain access to resources and assist them with managing personal and family issues.
Must provide assist clients and families in coordination of care and the management of a patient's individual needs through and beyond the course of oncologic treatment.
The social worker must possess the ability to assess, coordinate, plan, monitor, implement and evaluate the patient's mental health plan of care while ensuring and facilitating the achievement of quality clinical outcomes.
Must provide care management and care coordination for adult patients with mild to severe medical illness and oncologic diagnosis and oversee the coordination of services as well as the case management and navigation for this population.
This responsibility includes indigent pharmaceutical management coverage and approval.
The social worker collaborates with members of the health care team to empower patients to manage their disease process and assist patients, identified as at risk in all aspects of the care continuum including therapy services.
The social worker oversees the collaboration with the provider practice, the case management model as a whole, and implements, monitors, and evaluates delivery of individuals care plans to improve upon patient health status.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

Coordinates/facilitates patient progression throughout the continuum:
Addresses/resolves system problems impeding diagnostic or treatment progress; proactively identifies and resolves delays and obstacles to discharge Seeks consultation from appropriate disciplines/departments as required to expedite care and facilitate discharge Identifies at-risk populations using approved screening tool and follows established reporting procedures Collaborates and communicates with multidisciplinary team in all phases of discharge planning process, including initial patient assessment, planning, implementation, interdisciplinary collaboration, teaching and ongoing evaluation Coordinates and manages indigent drug coverage for oncology patients Coordinates support services for oncology patients Actively participates in clinical performance improvement activities Uses data and reports quality measure criteria Exceptional interpersonal skills with excellent written and verbal skills Coordinates transport and gift cards to support oncologic treatment Coordinates the oncology resource center and donations to the center Provides feedback on quality documentation to support the patient's medical condition in the patients' clinical records.
Participates in developing policies and procedures in accordance with regulatory agencies.
Investigates and resolves patient complaints, or refers unusual problems to the Chief Clinical Officer (CC).
Reports incidents via the hospital notification system (HNS).
Assists with development, implementation and evaluation of case coordination, procedures, and standards of practice.
Serves a physician, ancillary, and community services liaison between all members of the care team and the patient.
Participates in the Performance Improvement (PI) Process.
Develops a quality monitoring and performance indicator program for case management within the walls of the rural health clinics.
Demonstrates awareness of hospital policies and procedures, assumes responsibility for learning new policies policies and procedures.
Assumes responsibility for professional and personal development, self-learning needs are communicated to appropriate resources.
Updates clinical staff and medical staff on new standards as developed by third party payors and facilitates support for payer and payment related issues for patients.
